Athletics Permission Form Grade 5-9

PARENTS MUST SIGN A WAIVER ON POWERSCHOOL for athletics under School Fees and Forms, under athletics. 

• This is for all students in Grade 5-9 who want to play on a sports team.
• This must be completed from the parent account, not the student account. This is only accessible through a computer, not mobile device.

• All students must have a form to participate in open gyms and tryouts. 
• One form is will cover your student for volleyball, basketball, and badminton.
